All 17 enemies died; every death came after 1:16, when a second pack added two Storm Adepts and nine Lightning Bolts landed in 20 seconds against two kicks.
This key was not lost to a wipe or to the clock. It was one continuous pull that the group won: all 17 enemies died, the last at 3:02, and the log ends 16 seconds later without saying why the key was dropped. Even with 1:15 of death penalty, the in-game timer would have read about 4:08 of 32:00.
The pull was really two packs. The first, up to 1:16, had one Storm Adept and nobody died, but it used almost everything: Healing Tide, Heroism, Spirit Link (which pulled Hyporock up from 3% at 0:31), and Hyporock's Divine Shield, Divine Protection, Healthstone and potion. The second pack was pulled before the first was dead and had two Storm Adepts. Their Lightning Bolts hit anyone but the tank for 300,000-370,000, and Sandfury Stonefist's Ground Pound, a four-second slam that cannot be kicked and hits all five for 300,000-450,000, landed on top. Ground Pound is in all five death windows and was the killing blow in two.
Whose fault: nobody's alone. What broke was interrupt coverage once two casters were bolting at the same time, plus one arrow that can be sidestepped. Nobody was battle-rezzed: every death was a release and a run of about 180 yards back from the entrance, 16 to 22 seconds out of the fight. Figures that are not in the tables below were read straight from the combat log.
